Specific requirements to requesting a juvenile record are outlined below. These requirements must be met prior to the department processing your request. **
- Any person, agency, business or insurance company requesting a juvenile record must do so in writing.

The identity of the requester must be verified prior to any release of information to ensure they are entitled to receive the record.

Photo Identification of party requesting record, and
-
a Birth Certificate verifying the relationship with the child, or
-
a Court Order showing legal guardianship

Information will not be released if the case in question is an active or pending investigation or prosecution, and the release of that information would jeopardize the investigation/prosecution, or subject an individual to threats of harm or intimidation. If the requester is denied information under this policy, the requester should be referred to the appropriate agency for more information.
